Questions: This patient has a mild Class 2 Div 2 malocclusion with moderate lower crowding. Her main concern is her upper teeth. Space calculation reveals a discrepancy of 1.84. An upper Inman Aligner will be used to correct the position and a Clear Aligner will be used to finish off the case. There are old comp veneers on the UR2 and UL2 which will be changed and bonding will be applied to the incisal tips once the are straight. Although the patient is fine with the lowers, we may review it with her once the uppers are completed. Is this a case suitable for the Inman Aligner?
Answers: This is a good case. Even though you are moving the centrals forward and thus would be creating space and your discrepancy is small, I would still consider using an Expander on the Aligner because there is a fair degree of overlap with the laterals. I would aggressively strip away some of the old composite on the mesial of the laterals; they look slightly wide anyways. Once the incisors are aligned, I would widen the tip of the rotated canine with composite and use a Clear Aligner to give it a little twist immediately after to get it positioned nicely. Advancing the centrals my cause you to lose your anterior stop. I have done this countless times without any issue as long as lateral guidance is protected. After completing the upper teeth, you will be able to advance the lowers fairly easily if the patient wants.
