Question: This patient is concerned with the mild overlap between the UL1 and UR1 and the way the UR2 protrudes. The patient has an anterior openbite, hence the occlusion poses no limitations the movements of the incisors. What are your suggestions in regards to interproximal reduction?
Answer: This case is very straight forward and ideal. In regards to gaining space, there are three options: 1. IPR only. 2. Combined Inman and Expander and some IPR. This option tends to cause change much more quickly and requires less IPR and is the recommended choice. 3. Combined Inman and Expander and no IPR. This case may be possible without any IPR if the patient is totally against having any done, but remember you still need a slight amount to free up the contact to get the teeth moving. Composite anchors may be helpful to hold the bows at the incisal 1/3 and where to place them will be obvious upon fitting.
